Insurance
To provide you with the highest quality of healthcare, Prosper Natural Health does not contract with insurance companies. The medical insurance system has created a healthcare system where the doctor-patient interaction is rushed and the insurance company dictates medical treatment. You will never experience this at Prosper Natural Health. This means that our physicians do not bill insurance companies for the goods and services that you receive, and your payment is due at the time of service.

We are happy to provide you with the necessary codes and paperwork to submit to your insurance company for reimbursement. At the time of service, you may request a “superbill” which will have all the necessary coding information for your insurance carrier. Reimbursement rates vary, so please call your insurance provider to determine your coverage for a primary care physician who is an “out-of-network provider.”

Payment
Payment is always due at time of service. We accept cash, checks, Visa, or MasterCard. Outstanding balances will be charged to the credit card on file 30 days after initial invoice. Apothecary products once sold cannot be returned by state law.

Missed Appointment Fee & Initial Appointment Deposit
Payment is accepted by check, cash or credit card and is required at time of service for established patients.

Established clients are charged a Missed Appointment Fee of $100 for any missed appointments or late cancellations (less than 24 hours notice).

New patient appointment fees are collected at the time of booking to hold your appointment slot. New patient appointments can be rescheduled within 6 months of the original appointment date. The fees for new patient appointments are non-refundable. Five business days prior to being seen by the doctor, you are required to have submitted all online intake forms and ensure we have received your appropriate medical records to allow for your doctor to review your records. In the event that intake forms are not completed five business days prior to your new patient appointment, your appointment rescheduled.
