New Patient Intake Forms

If you are a new patient, please complete the digital forms emailed to you at least 5 business days before your visit. This allows your doctor to review your records prior to your appointment.  The questionnaire links emailed to you are the required comprehensive intake method which will ensure a more productive visit with us.  If you do not have access to completing the online these questionnaires, please contact our patient care staff at (360)385-5375.

In the event that intake forms are not completed five business days prior to your new patient appointment, your appointment will be canceled without refund of your deposit.
