Five business days prior to being seen by the doctor, you are required to have submitted all online intake forms and ensure we have received your appropriate medical records to allow for your doctor to review your records. In the event that intake forms are not completed five business days prior to your new patient appointment, your appointment will be rescheduled.

2. Bring In Medications. If you are taking supplements or other medicines, please bring them to your first appointment.

3. Bring In Records. If you have copies of your most recent lab work or imaging, please bring a copy to the visit.

4. Prepare With Notes. Write down your questions and concerns prior to the visit to ensure that they are answered.

Your first visit

Initial appointments are typically between 60-120 minutes long. During this visit, you and your doctor will discuss your health concerns and you will be given the opportunity to share you medical history. Your doctor will perform a physical exam and, if necessary, discuss tests that would be useful in clarifying your condition and needs. At this visit, you will receive a customized treatment plan specifically for you. This plan is designed to help correct your underlying imbalances and to move you towards your optimal health.

Your return visits

Return visits are typically 30-60 minutes in length. These visits are designed as check-ups to see how you’re doing, note positive changes, and make adjustments to your personalized treatment plan. To best suite your needs, a physical exam maybe performed at this visit. Your doctor may recommend further testing, depending on your personal needs and condition.

Contacting Our Physicians

If you are an existing patient, the best way to contact our doctors regarding your treatment is by leaving a message through your patient portal or by talking with our staff during business hours at (360)385-5375. You may also leave a message on our confidential voicemail.

Portal messages that relate to a new problem or non-recent (within the last 2 weeks) visit, will require a brief telephone appointment or standard appointment.

Email is not a dependable or secure form of communication for healthcare matters so our office asks that you communicate by phone or via the secure patient portal for all existing patient care matters.

Cancellation & missed appointments

We ask that you give us at least 48 hours notice if you need to cancel or reschedule your appointment. If you cancel your appointment without 24 hours notice or miss your appointment, you will be charged a $100 fee.
